Key Responsibilities:
As an RN Clinical Manager, your role will encompass:
1. Managing patient referrals and ensuring timely assignments of clinicians and therapists for evaluations.
2. Overseeing the determination of patient eligibility for home health benefits and ensuring ongoing insurance approvals.
3. Continuously assessing patient needs and ensuring that care is tailored to individual requirements, compliant with physician directives.
4. Supervising the development and implementation of personalized care plans, making updates as necessary.

Qualifications:
- Current RN licensure in the state of practice
- Valid CPR certification
- Current Driver's License and vehicle insurance
- Access to reliable transportation
- Previous experience in home health is highly preferred
