Position Summary: As a Case Manager at GLSS, you will work closely with elderly individuals and those with disabilities to facilitate their access to essential services, enabling them to maintain their independence. Your role will involve assessing their needs, informing them about available resources, and aiding them in obtaining the necessary services.


Key Responsibilities: Conduct thorough in-home evaluations of clients' overall circumstances to identify needs and develop personalized care plans. This includes identifying, coordinating, authorizing, securing, and continuously monitoring community-based services. Adapt and revise care plans as clients' needs or situations evolve.


Qualifications:

• Experience in the human services sector or a Bachelor's degree preferred

• Bilingual in Spanish is mandatory

• Strong computer proficiency

• Excellent time management skills

• A genuine passion for helping others and the ability to build positive, professional relationships

• Advocacy skills for individuals requiring services and support

• A sincere commitment to assisting others

• Attention to detail

• Capability to manage confidential and sensitive information appropriately

• Reliable transportation for home visits within the GLSS service area

Work Schedule:
Full Time, 40 hours per week (Monday-Friday) with an option for a 4-day work week.
