Job Title: Clinical Team Lead - End-of-Life Services

Overview
In our mission to deliver exceptional end-of-life care, we are seeking an experienced Clinical Team Lead to oversee hospice operations and provide guidance to clinicians. This role is ideal for a registered nurse with strong leadership skills and a passion for promoting compassionate care delivery.

Responsibilities
• Collaborate with the interdisciplinary team to coordinate patient care and ensure continuity of services.
• Provide real-time support and mentorship to clinicians to promote effective performance and high-quality care delivery.
• Conduct home visits, conferences, and record reviews to assess patient needs and adjust care plans accordingly.
• Support staff development by setting therapeutic goals and providing guidance on best practices in hospice care.
• Work closely with medical teams to coordinate smooth transitions and maintain continuity of care for referred patients.

Requirements
• Registered nurse with current licensure to practice professional nursing in the state.
• Previous experience in hospice/home care setting and two years of management or supervisory experience.
• Excellent observation, communication, and leadership skills.

Salary Range: $85,000 - $105,000 per annum, based on experience and qualifications.
