Job Summary:
Mcare Agency LLC is seeking a skilled Physical Therapist Assistant to join our team in providing high-quality patient care services in a home health setting.
Key Responsibilities:
- Deliver quality care to patients in their homes, focusing on helping them attain maximum functional levels.
- Assess and interpret patient evaluations and test results to determine physical therapy treatment plans in consultation with physicians.
- Document patient care services by charting in patient and department records, ensuring compliance with federal, state, and local legal and professional requirements.
- Follow up with home health orthopedic patients, ensuring continuity of care and meeting eligibility requirements for home health services.
- Perform maintenance therapy and other duties as assigned.
Requirements:
- Graduation from an accredited Assistant Therapy Assistant Program.
- In-state licensure as a Physical Therapist Assistant.
- At least 1 year of home health experience as a Physical Therapist Assistant.
- Strong caring interpersonal skills and a valid driver's license with a clean DMV record.
