Physical Therapist Assistant Home Health Specialist

Medi Home Health & Hospice is seeking a Part-time / PRN Physical Therapist Assistant (PTA) to provide physical therapy services to patients in their homes in State College, PA.
- Collaborate with the physical therapist to develop and implement treatment plans for patients.
- Assist patients with physical therapy exercises and activities to improve their mobility and function.
- Monitor patient progress and report to the physical therapist and other healthcare team members.
- Develop and maintain accurate and timely client care progress records.
- Communicate effectively with patients, families, and other healthcare team members to ensure seamless care.
Requirements
- Valid PTA license in the state of PA.
- Valid Driver's License.
- Minimum of one-year verifiable physical therapy experience.
- Home Health experience preferred.
