Job Summary

Northeast Center for Youth & Families, Inc. (NCYF) is seeking two full-time Clinical Case Manager/Educational Liaisons (CCM) to join our Residential Services team. As a CCM, you will provide clinical, collateral case management, ongoing assessments, and aftercare to our youth. You will also act as an Educational Liaison to support the youths' learning and ensure their Individualized Education Programs (IEPs) are comprehensive and meeting their needs in the school environment.

Key Responsibilities
  • Act as an advocate for the youth in the educational setting
  • Attend school meetings and establish relationships with students and provide for needs that emerge
  • Provide on-call support to the programs
  • Collaborate with the treatment team to develop and implement individualized treatment plans
  • Conduct ongoing assessments and provide case management services to ensure the youth's needs are met
Requirements
  • A Master's Degree is required
  • A valid driver's license with approved DMV, DCF, and CORI background checks
  • Travel is involved and a reliable vehicle is needed
