Position Overview:
The Patient Care Assistant (PCA) works under the supervision of the Registered Nurse (RN) Supervisor and Licensed Practical Nurse (LPN) to deliver individualized patient care tailored to specific client requirements, adhering to established nursing protocols and standards.
Key Responsibilities:
The PCA will be accountable for the following tasks:
1. Support nursing staff in delivering patient care, including assistance with personal hygiene, meal preparation, and bathroom needs.
2. Aid in gathering assessment data and accurately documenting findings in patient records, which may encompass vital signs, weight, blood glucose levels, and specimen collection.
3. Contribute to the formulation of the patient care plan based on identified needs and concerns.
4. Execute basic nursing procedures and follow directives from RN and/or LPN with precision.
5. Identify and promptly report any changes in patient conditions or behaviors.
6. Provide care while adhering to infection control protocols and safety measures.
7. Utilize safety equipment correctly, including side rails, alarms, and restraints as prescribed.
8. Implement seizure precautions as necessary.
9. Report and rectify unsafe conditions immediately, such as wet floors or loose handrails.
10. Maintain proper body mechanics during all patient care activities.
11. Ensure safety when operating medical equipment like feeding pumps and oxygen tanks.
12. Accompany patients to medical appointments and social activities as required.
13. Assist patients in enhancing their activity levels.
14. Position patients correctly using appropriate support devices.
15. Perform basic range of motion exercises and reinforce prescribed exercises.
16. Assist patients with mobility and transfers based on their activity levels.
17. Report any signs of activity intolerance, such as shortness of breath or increased discomfort.
18. Assist with patient admissions, transfers, and discharges as instructed.
19. Participate in mandatory training sessions and staff meetings as scheduled.
20. Collaborate effectively with other healthcare team members to ensure coordinated patient care.
21. Help maintain a clean and safe working environment.
22. Drive patients to appointments and activities as directed by RN Supervisor or LPN.
23. Perform additional duties as assigned.
