Physician - Family Medicine Specialist

Unity Health Care seeks a dedicated and compassionate Family Medicine Physician to join our team in providing high-quality healthcare services to vulnerable populations. As a key member of our healthcare team, you will be responsible for delivering culturally sensitive care, collaborating with colleagues, and contributing to the organization's mission to improve health outcomes, patient experience, and care team well-being.
Key Responsibilities- Provide comprehensive primary care services to patients, including diagnosis, treatment, and prevention of health conditions.
- Collaborate with healthcare teams to develop and implement patient care plans, ensuring continuity of care and optimal health outcomes.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices and policies.
- Communicate effectively with patients, families, and colleagues, promoting a patient-centered approach to care.
- Stay up-to-date with the latest medical research, guidelines, and best practices, applying this knowledge to improve patient care and outcomes.
- Current DC Professional License and CDS and DEA registrations (if applicable).
- Graduate of an accredited school of medicine or nurse practitioner, physician assistant, or other relevant professional school.
- Board eligible or board certified/ licensed in area of specialty, with a commitment to ongoing professional development.
- Ability to work collaboratively as part of a multidisciplinary team, with a focus on patient-centered care and population health management.
