Job Summary

We are seeking a skilled Physical Therapist to join our team at Fortis Health. As a Physical Therapist, you will provide high-quality physical therapy services to patients in their homes, utilizing evidence-based practices and treatment plans.

Responsibilities
  • Provide skilled physical therapy interventions and treatments to patients in their homes, in accordance with physician orders and treatment plans.
  • Assess and evaluate patients' level of functioning, and develop and revise treatment plans as needed.
  • Treat patients to relieve pain, develop and restore function, and maintain maximum performance, using appropriate physical therapy treatments, modalities, and procedures.
  • Observe, record, and report patients' reactions to treatment and any changes in their condition to physicians, DONs, and Case Managers.
  • Educate patients, caregivers, and healthcare teams on physical therapy phases, prosthetic and orthotic devices, and durable medical equipment, as appropriate.
  • Provide staff in-service education, as directed by DOR/DON.
  • Assess and regularly reevaluate patients' needs, and develop, implement, and revise treatment plans accordingly.
  • Initiate diagnostic, preventive, and rehabilitative procedures, as appropriate, to ensure good coordination of care.
  • Promote coordination of care through interdisciplinary case conferences and communication with physicians and field staff.
  • Develop, prepare, and maintain accurate and timely individualized patient care progress records, in accordance with agency policy and procedures.
  • Participate in the agency's quality improvement program, and stay up-to-date with physical therapy trends and knowledge for service provision, documentation, and care coordination.
  • Supervise Physical Therapy Assistants, as needed.
Qualifications
  • Successful completion of a physical therapy program at a school of physical therapy approved by the American Physical Therapy Association or the Council on Medical Education and Hospitals of the American Medical Association, or both.
  • Licensed as a physical therapist.
  • No minimum experience required; we welcome newly graduated physical therapists.
