About the Role

We are seeking a highly skilled Master's level Social Worker to join our hospice team at Gentiva Health Services. As a key member of our interdisciplinary team, you will provide psychosocial support to patients and their families, utilizing your professional training and personal judgment to monitor the psychosocial assessment process.

Responsibilities
  • Provide comprehensive psychosocial assessments to identify the needs, goals, interventions, and services required for patients and their families.
  • Develop and implement individualized plans of care that address the physical, emotional, and spiritual needs of patients and their families.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
Requirements
  • Master's degree in Social Work from a CSWE-accredited school.
  • Licensure as a Social Worker in the state where the hospice is located.
  • At least one year of experience as a Social Worker in a healthcare or hospice setting, with three to five years preferred.
