About the Role

This 13-week assignment requires a skilled nurse to work as a case manager, collaborating with healthcare teams to assess, plan, facilitate, and coordinate patient care. You will oversee the effective utilization of services and resources, promoting patient safety, quality of care, and cost-effective outcomes for patients requiring long-term care or home care after hospitalization.

Responsibilities
  • Collaborate with healthcare teams to develop and implement patient care plans
  • Assess patient needs and coordinate services to ensure seamless transitions
  • Communicate effectively with patients, families, and healthcare professionals to promote quality care

Requirements
  • At least 1 year of recent acute care experience in specialty
  • Active RN license
