Key Responsibilities- Develop and implement personalized physical therapy treatment plans to help patients achieve their goals and improve their quality of life.
- Conduct thorough assessments and evaluations to identify patients' strengths and weaknesses, and develop targeted interventions to address their needs.
- Collaborate with our interdisciplinary care team, including physicians, nurses, and social workers, to ensure comprehensive and coordinated care.
- Provide education and training to patients and their families on proper techniques and strategies for managing their condition and promoting optimal health outcomes.
- Document patient progress and outcomes, and participate in regular team meetings to discuss patient care and make adjustments to treatment plans as needed.
- Master's degree in Physical Therapy from an accredited program.
- Current and unrestricted Physical Therapy license.
- Minimum of one year of experience in home health or a related field.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Valid driver's license and reliable transportation.
