Job Description

Position: RN Case Manager

Reports to: Director of Patient Care

Department: Patient Care Services

FSLA Status: Exempt

Job Summary: The RN Case Manager plays a vital role in orchestrating the care of designated patients. This includes conducting thorough evaluations of patients' physical, emotional, and psychological states, delivering skilled interventions, and providing guidance to patients and their families to ensure optimal symptom management and comfort. The RN Case Manager serves as a key liaison among all healthcare professionals involved in patient care.

Supervises: Patient Care

Required Qualifications:

Knowledge/Skill/Ability:

  • Ability to assess and respond effectively to the needs of patients and families in emotionally charged and stressful situations.
  • Proficient in assessment, intervention, and communication strategies.
  • Capable of clearly explaining and teaching information related to hospice and end-of-life care.
  • Comprehensive understanding of hospice admission criteria.
  • Ability to create and modify care plans tailored to individual patient needs and objectives.
  • Awareness and acceptance of diverse cultures, values, and healthcare approaches regarding dying and loss.

Equivalent Education/Experience:

  • Current RN licensure in the state of Oklahoma; a Bachelor’s degree is preferred.
  • A minimum of one year of experience in a role that demonstrates assessment and intervention capabilities.
  • Prior experience in home health or hospice settings is preferred.

Certifications/Licenses:

  • Valid Oklahoma Driver's License and proof of automobile insurance.
  • Current state registration and licensure to practice as a registered nurse in Oklahoma.
  • Certification as a Hospice and Palliative Care Nurse (CHPN) is preferred.

Key Responsibilities/Essential Functions:

  • Coordinates comprehensive nursing care for a caseload of 10-12 patients, considering physical, psychological, social, and spiritual factors affecting health.
  • Facilitates care conferences with patients and families, educating them about disease processes and care administration.
  • Collaborates with attending physicians, community organizations, and hospice staff to optimize resources for patients and their families.
  • Ensures continuity of care through active participation in daily reports, staff meetings, and interdisciplinary group meetings.
  • Documents and communicates relevant data regarding patient care in accordance with agency policies.
  • Engages in interdisciplinary group meetings for the development and review of care plans, problem-solving, and professional support.
  • Participates in on-call duties as needed.
  • Assumes care responsibilities in the absence of other RN case managers as directed.
  • Contributes to orientation and in-service training programs.
  • Engages in agency and departmental quality improvement, marketing, fundraising, and public relations efforts.
  • Organizes daily tasks to meet patient needs while ensuring efficient use of time and resources.
  • Additional Responsibilities:
    • Meets expectations for approximately 20 patient visits weekly, with adjustments for part-time case managers.
    • Conducts supervisory visits for home health aide services as per agency policy.
    • Obtains follow-up orders for consulting services such as nutrition and therapy.
    • Performs other related duties as assigned within the scope of competencies.
    • Maintains any provided equipment, such as laptops and phones, ensuring they are returned in working condition upon termination of employment.
