RN - Transitional Care Float Pool (Clackamas / OnCall)

As a Registered Nurse in our Transitional Care Float Pool, you will play a vital role in providing high-quality patient care in a dynamic and fast-paced environment. Under the general supervision of a healthcare team, you will use your clinical judgment skills to diagnose and treat human responses to actual or potential health problems.
Key Responsibilities:- Assessment: Perform accurate multi-system assessments, including biophysical, psychosocial, environmental, self-care, education needs, and discharge planning needs. Identify deviations from normal on routine lab diagnostic tests and reassess patients throughout their hospitalization.
- Planning: Develop an individual nursing plan of care that is sensitive to the developmental needs of the patient. Coordinate patient care activities and discharge planning with other hospital and clinic departments and community agencies. Develop patient teaching plans and document them on the care plan or teaching plan.
- Implementation: Initiate appropriate measures based on protocol for the management of medical and/or surgical patients. Carry out the physician's plan of care accurately and document it in the electronic medical record. Utilize a multidisciplinary approach when appropriate and perform all necessary procedures correctly.
- Evaluation: Evaluate assessment methods, plan of care, and patient response to treatment. Participate in unit quality assurance activities and contribute to the improvement of patient care.
- Minimum 9 months of RN experience
- 2 years of recent experience in a progressive or intermediate care unit
- Graduate of an accredited nursing program
- Registered Nurse License (in the state where care is provided) required at hire
- NIH Stroke Scale Certificate within 3 months of hire
- Basic Life Support required at hire
- Previous healthcare experience
- Bachelor's degree in nursing or related field
- Recent experience with respiratory compromised patients, including long-term ventilation, tracheostomy, high-flow nasal cannula, and BiPAP
- PCCN/CCRN certification
This is a variable shift position with a call-in/on-call requirement. The work environment is fast-paced and dynamic, with a focus on providing high-quality patient care.
