We are currently seeking empathetic and committed Licensed Practical Nurses to become part of our team in New Jersey. The selected candidates will be responsible for delivering personalized care and services to patients as required. A valid LPN License in the state of New Jersey is essential for consideration.

This role involves working from 7PM to 8AM, providing 1:1 care for patients.

Key Responsibilities:

  • Provide care for patients in their designated locations as needed.
  • Assist patients with daily living activities.
  • Adhere to care plans and ensure the highest standard of care is delivered.
  • Document medical information accurately as required.
  • Collaborate effectively with the interdisciplinary team.
  • Maintain open communication with patients and their families.

Qualifications:

  • Valid LPN License in New Jersey.
  • Previous experience in hospice care is advantageous.
  • Possession of a valid driver’s license with auto insurance.
  • Access to reliable transportation.
