Job Summary:

We are seeking an experienced Onsite Health Engagement Nurse to promote engagement in clinical/disease management programs and improve member healthcare decisions.

Key Responsibilities:

  • Educate members on health education and condition management, collaborating with OptumHealth.
  • Refer employees to available clinical programs, including premium designated physicians.
  • Create and implement employer-specific programs based on employer needs.
  • Monitor and analyze customer-specific health analytic data, developing targeted data-driven strategic plans and programs.
  • Provide clinical interface with facilities and providers for issue resolution, benefit interpretation, and program clarification.
  • Knowledge of community resources for referral and community outreach.

Requirements:

  • Registered Nurse with active and unrestricted license for state of residence.
  • 3+ years of clinical experience in a hospital, acute care, home health/hospice, direct care, or care management.
  • Proficient in Microsoft Word, PowerPoint, and Excel, including navigation of a Windows environment.
  • Proven presentation skills with experience delivering group presentations and 1:1 coaching.

Preferred Qualifications:

  • Bachelor's degree in Nursing.
  • 3+ years of experience in managed care with significant program management experience.
  • Wellness/disease management, program promotion, education, and development experience.
  • Diabetes education experience.
  • Case management experience and/or Case Management Certification (CCM).
  • Community outreach experience.
  • Familiarity with data analysis (utilization/program outcomes) and reporting.

Soft Skills:

  • Excellent verbal communication skills.
  • Self-starter; ability to work with senior leadership.
