The LPN Medication Care Manager is a vital member of our care team, responsible for providing high-quality medication management services to our residents. This role requires a strong commitment to delivering exceptional care and ensuring the safety and well-being of our residents.
Responsibilities- Administer medications and treatments in a safe and organized manner
- Document medication administration and maintain accurate records
- Conduct Service and Health Updates as directed by the Resident Care Director
- Participate in the development of Individualized Service Plans (ISPs) and monthly updates
- Transcribe orders and maintain accurate documentation
- Partner with the community team to ensure compliance with OSHA and Occupational Health and Safety Regulation requirements
- Report all accidents, incidents, and unsafe conditions immediately
- LPN, LVN, or state/province specific licensed nurse credential
- CPR and First Aid certification
- Ability to make choices and decisions in the best interest of residents
- Ability to react and remain calm in difficult situations
- Ability to handle multiple priorities and maintain accurate documentation
- Written and verbal communication skills for effective communication
- Good judgment, problem-solving, and decision-making skills
