LPN Visit Nurse Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Almost Family, part of LHC Group. As a LPN Visit Nurse, you will provide high-quality clinical services to patients in their homes, working under the direct supervision of a Registered Nurse and/or designated supervisor.

Key Responsibilities:
  • Provide direct nursing care to patients in their homes, following the plan of care and in coordination with other members of the healthcare team.
  • Document patient visits thoroughly and completely, adhering to regulatory and payer requirements.
  • Participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition.
  • Ensure confidentiality of patient health information and maintain a professional demeanor at all times.
Requirements:
  • Current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
Preferred Qualifications:
  • Previous home care experience.
