We are seeking a skilled LPN to join our team at Almost Family, part of LHC Group, where we prioritize a culture of caring, belonging, and trust. As a Home Care Licensed Practical Nurse, you will be responsible for providing direct nursing care to patients in their homes, following a plan of care and in coordination with other healthcare professionals. Your duties will include:

  • Providing high-quality clinical services within the scope of practice, as defined by state laws governing nursing practice.
  • Documenting patient visits thoroughly and completely in the electronic medical record.
  • Actively participating in care coordination, including timely reporting of patient needs and changes in condition.
  • Ensuring confidentiality of patient health information.

To be successful in this role, you will need:

  • Current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
