About the Role

We are seeking a skilled Speech Language Pathologist to join our team at SpringHill Home Health and Hospice. As a Home Health Therapist, you will work with patients in their homes, providing essential one-on-one care and support to help them achieve their maximum level of speech, language, cognitive, and swallowing abilities.

Responsibilities
  • Perform comprehensive evaluations and develop individualized treatment plans for patients with speech, language, cognitive, and swallowing disorders.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and effective communication with patients, families, and healthcare providers.
  • Conduct regular assessments and re-evaluations to monitor patient progress and adjust treatment plans as needed.
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal care outcomes.
  • Maintain accurate and timely documentation of patient evaluations, treatment plans, and progress.
Requirements
  • Master's degree in Speech Language Pathology from an accredited program.
  • Current, unrestricted license as a Speech Language Pathologist in the state of practice.
  • Basic Life Support or cardiopulmonary resuscitation certification.
  • Minimum one year of experience as a Speech Language Pathologist.
  • Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
