We are seeking a skilled Licensed Practical Nurse to join our team at RTW Renal Treatment Centers - West, Inc. as a Renal Care Specialist. Responsibilities- Provide direct patient care in an outpatient dialysis setting, including administering medications, monitoring vital signs, and performing other nursing tasks as needed.
- Collaborate with the interdisciplinary care team to develop and implement individualized patient care plans.
- Conduct thorough assessments of patients' physical and emotional needs, and make recommendations for care and treatment.
- Participate in ongoing education and training to stay current on best practices and advancements in renal care.
- Contribute to a positive and supportive team environment, promoting open communication and a culture of excellence.
- Current LPN-LVN license in the state of practice required.
- Current CPR certification required.
- IV certification required if required by state regulations.
- High school diploma or equivalent required.
- Completed training program approved by the medical director and the governing body as required by CMS guidelines prior to operating the water treatment system.
- BONENT or CHT certification preferred or as required by state regulation.
- Minimum of one year's experience in a hospital or clinical setting preferred.
- Basic computer skills and proficiency in MS Word and Outlook required; functional proficiency in computer systems of DaVita within 90 days of hire required.
