Position Overview

The Advanced Practice Nurse or Clinical Nurse Specialist is accountable to the Executive Director and plays a crucial role in delivering strategic insights, organizational structure, and evidence-based evaluations to facilitate informed decision-making and fulfill organizational objectives. This position necessitates the embodiment of core values such as trust, empathy, commitment, humility, creativity, and a focus on community.

As a vital member of the primary care team, you will conduct annual health evaluations, perform post-discharge follow-ups, and provide high-quality, cost-effective care to designated patients. Collaborating within a multi-disciplinary team, you will aid in the development, execution, and continuous enhancement of home health procedures.

Key Responsibilities

The Advanced Practice Nurse or Clinical Nurse Specialist will be tasked with the following:

  • Conducting thorough in-home evaluations for designated patients and supporting the primary care team in managing chronic conditions.
  • Assessing current symptoms and determining diagnoses for effective care management and medical treatment.
  • Performing physical assessments, medication evaluations, vital sign checks, laboratory draws, wound management, and mental health screenings.
  • Communicating assessment results to the primary care provider (PCP) to highlight potential care gaps.
  • Ensuring precision in risk assessment, high-quality metrics, and closure rates.
  • Achieving elevated patient satisfaction and enhanced health outcomes, including decreased hospitalizations and emergency room visits.
  • Employing critical thinking to address patient needs, providing suitable resources, and follow-up care to enhance stability.
  • Educating patients on disease management, medication adherence, and compliance.
  • Adhering to all HIPAA regulations and safeguarding the confidentiality of protected health information.
  • Traveling regularly to deliver care to patients in their residences based on organizational needs.
