Job Title: Manager of Assessments for Care Management Services

At CHSGa, we're committed to delivering the highest level of care in our communities. As a Manager of Assessments for Care Management Services, you'll play a vital role in ensuring our patients receive the best possible care.

Key Responsibilities:
  • Lead the assessment process for potential members, ensuring timely and effective care management.
  • Collaborate with leaders across departments to ensure seamless admissions and transitions.
  • Develop and implement best practices for assessment and admissions, supervising LPN staff to ensure high-quality care.
  • Work towards departmental goals that align with our organization's strategic objectives.
  • Oversee all aspects of the admission process for assigned care management operational regions.
  • Ensure adequate training for LPNs and RNs, promoting a culture of excellence in care management.
  • Coordinate new assessment appointments within required timeframes, ensuring efficient patient flow.
  • Work with the Health Care Navigation team to ensure smooth transitions of patients in the CHSGa system.
  • Participate in team meetings and contribute to the success of our organization.
Requirements:
  • RN license with a minimum of 5 years of experience in the health and human services field, with 5 years of supervisory experience.
  • Or, Bachelor's degree in Social Work or a related field with a minimum of 5 years of experience in the health and human services field, with at least 5 years in a management or supervisory position.
  • Experience in social work, home and community-based services, healthcare, or geriatrics is preferred.
  • Experience with standard assessment tools, MDS, RAI is also preferred.
  • Valid Driver's License and reliable transportation are required.
