VNS Health is seeking a skilled Social Work professional to join our team in Bronx, New York.

Key Responsibilities
  • Deliver outreach services to individuals in need, providing in-home mental health intervention and connecting clients to vital resources.
  • Collaborate with colleagues to assess mental health service needs and develop short-term service plans for clients.
  • Respond promptly to dispatch calls and ensure seamless program operations as a liaison to community agencies.
  • Provide comprehensive care, including counseling, care management, and long-term supplemental treatment referrals.
  • Prepare case histories and maintain accurate records, in accordance with program requirements.
Requirements
  • Current registration to practice as a Licensed Master Social Worker in New York State.
  • Valid driver's license may be required.
  • Minimum of one year experience as a Social Worker in a healthcare setting.
  • Master's degree in social work from a graduate school of social work accredited by the Council on Social Work Education and the Education Department.
