Senior Hospice Care Specialist

We are seeking a compassionate and skilled Senior Hospice Care Specialist to join our team at LHC Group in Columbus, OH. This is a full-time position that requires home visits for hospice patients. The ideal candidate will have a Bachelor's degree in social work or a related field and be licensed as a Social Worker in Ohio.
This role will also involve volunteer management duties as needed. The Senior Hospice Care Specialist will provide medical social services under the direction of a physician and interdisciplinary group to assist patients and their families in understanding the impact of social and emotional factors on their health status. This may include assessing patients' personal, emotional, environmental, and social needs and developing coping mechanisms. Additionally, the Specialist will contribute to the development of a comprehensive Plan of Care and instruct healthcare team members on community resources available to patients.
The ideal candidate will have a strong understanding of bereavement needs and be able to establish a Plan of care that addresses these needs, including clear delineation of services and frequency of service delivery. This role also requires the ability to function as a Bereavement Coordinator and supervise the provision of bereavement services.
Key qualifications for this role include a Bachelor's degree in social work or a related field, one year of experience in a healthcare setting, current CPR certification, a valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation. Additionally, licensure as a Social Worker from the State Board of Social Work in Ohio is required.
We offer a competitive salary range of $63,000 - $85,000 per year, depending on experience. This estimate is based on national averages for social workers in Ohio.
