Key Responsibilities- Provide direct patient care, including assessment, planning, implementation, and evaluation of patient care.
- Collaborate with the healthcare team to develop and implement individualized patient care plans.
- Conduct preliminary assessments of patients to identify potential health risks and develop strategies for prevention and intervention.
- Evaluate patient responses to care and make adjustments as needed to ensure optimal outcomes.
- Document patient information accurately and thoroughly, including progress notes and discharge summaries.
- Participate in quality improvement initiatives to enhance patient care and safety.
- Develop and implement educational programs for patients and families to promote health and wellness.
- Current licensure as a Registered Nurse in the state of employment.
- Basic Life Support (BLS) certification.
- 2+ years of experience as a Registered Nurse in an ambulatory setting.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Bachelor's degree in Nursing.
- Experience working with geriatric patients.
