  • Collaborate with the healthcare team to develop and implement individualized patient care plans.
  • Conduct preliminary assessments of patients to identify potential health risks and develop strategies for prevention and intervention.
  • Evaluate patient responses to care and make adjustments as needed to ensure optimal outcomes.
  • Document patient information accurately and thoroughly, including progress notes and discharge summaries.
  • Participate in quality improvement initiatives to enhance patient care and safety.
  • Develop and implement educational programs for patients and families to promote health and wellness.
Requirements
  • Current licensure as a Registered Nurse in the state of employment.
  • Basic Life Support (BLS) certification.
  • 2+ years of experience as a Registered Nurse in an ambulatory setting.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
Preferred Qualifications
  • Bachelor's degree in Nursing.
  • Experience working with geriatric patients.
