As a RN Case Manager, you will conduct in-home nursing visits for our clients, providing physical assessments, ensuring home safety, and overseeing skilled and non-skilled caregivers. You will also provide case management to assigned clients, directing a team of RNs, LPNs, and caregivers in the delivery of care.
Key Responsibilities:- Conduct in-home nursing visits for clients
- Provide physical assessments, including vitals and home safety evaluations
- Oversee skilled and non-skilled caregivers providing care in the client's home
- Provide case management to assigned clients
- Direct a team of RNs, LPNs, and caregivers in the delivery of care
- Graduate of a state-approved school of professional registered nursing
- Current, unrestricted RN license in the state(s) of practice
- Minimum of two (2) years nursing experience, including one (1) year in home care or a closely related field
- One (1) year of supervisory and/or case management experience preferred
- $42.00 hourly rate, plus shift differentials
- Medical, dental, and vision benefits
