Job Summary:We are currently seeking a compassionate and skilled Hospice Aide (CNA) to provide direct patient care and support to our patients and families facing terminal illness. As a Hospice Aide (CNA), you will work closely with our interdisciplinary team to deliver high-quality, patient-centered care in the comfort of our patients' homes.
Responsibilities:- Provide direct patient care, including personal care, hygiene, and mobility assistance.
- Assist with daily living activities, such as bathing, dressing, and grooming.
- Document patient care and progress accurately and thoroughly.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Provide emotional support and companionship to patients and families.
- High school diploma or equivalency.
- Successful completion of a Board of Nursing approved training and competency evaluation program.
- Current listing with the Division of Facility Services Registry.
- One year of patient care and record keeping experience in an acute care or custodial setting.
- Documented proof of at least 75 certified education hours.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
