Care Coordinator, Utilization Management Specialist

Hackensack Meridian Health is seeking a skilled Care Coordinator, Utilization Management to join our team. As a key member of our healthcare team, you will be responsible for coordinating, communicating, and facilitating the clinical progression of patient treatment.
Key Responsibilities- Coordinate and communicate with patients, families, and healthcare teams to ensure seamless care transitions.
- Conduct thorough assessments of patient needs and develop effective care plans to optimize resource utilization.
- Collaborate with multidisciplinary teams to identify and address potential barriers to care, promoting efficient length of stay and resource utilization.
- Ensure compliance with regulatory and payor requirements, as well as state of New Jersey regulations for nursing.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and outcomes.
- Bachelor's degree in Nursing (BSN) or currently pursuing BSN with completion within 3 years.
- Effective decision-making and problem-solving skills, with the ability to influence and lead cross-functional teams.
- Excellent verbal, written, and presentation skills, with moderate to expert computer proficiency.
- Familiarity with hospital resources, community resources, and utilization management principles.
- NJ State Professional Registered Nurse License and AHA Basic Health Care Life Support HCP Certification required.
- Care Management certification by a nationally recognized organization within 1 year.
