Job Summary

We are seeking a skilled Nurse Practitioner to join our team at LifeBridge Health. As a key member of our interdisciplinary team, you will provide clinical expertise in hospice and palliative care, working collaboratively with our IDG to ensure high-quality patient care.

Responsibilities

  • Deliver patient-centered care in accordance with our mission statement and SPIRIT values
  • Collaborate with the IDG to develop and implement individualized care plans
  • Provide consultation and expertise in the delivery of care
  • Participate in education and training programs to enhance clinical skills and knowledge

Requirements

  • Master's degree with NP license required
  • 2-4 years of prior experience in palliative care or geriatric care preferred
  • Fleet Safety certification required
  • License to practice as a Nurse Practitioner in Maryland
