Palliative Care Nurse Specialist

Job Summary: We are seeking a highly skilled Palliative Care Nurse to join our team at LifeBridge Health. The successful candidate will have specialized training and expertise in navigating the healthcare system and resources for high-risk individuals in the community.
Key Responsibilities:
- Provide education and information to patients and their caregivers/family
- Work with the healthcare team and patients' physicians to develop a plan of care
- Identify individuals with chronic or life-limiting illnesses and/or at high risk for readmission to the ED or inpatient hospital
- Manage patients' care in coordination with their physicians and care team using established clinical pathways
- Educate and empower patients to take an active role in managing their disease process
Requirements:
- Associate's Degree or Bachelor's Degree in Nursing
- Minimum of 3 years experience working in an inpatient or outpatient healthcare setting
- 2-4 years experience working as an RN in a palliative/hospice care inpatient or outpatient setting preferred
- Reliable transportation
- 10 contact hours of continuing education per year
- End of Life Nursing Education Consortium Training (ELNEC) preferred
Preferred Qualifications:
- Advanced Nursing certification
- Knowledge of adult learning competencies and patient teaching skills
- Self-management skills reflecting self-motivation, initiative, and fiscal responsibility
