LPN Wellness Nurse

Sunrise Senior Living is a leading provider of senior care services, and we are seeking a highly skilled LPN Wellness Nurse to join our team. As a Resident Care Specialist, you will play a critical role in ensuring the health and well-being of our residents.
Responsibilities:
- Conduct regular wellness visits for all residents to assess their health and identify any potential issues.
- Develop and implement individualized care plans to meet the unique needs of each resident.
- Collaborate with the Resident Care Director to ensure that all residents receive high-quality care and services.
- Monitor and report any changes in a resident's health status to the Resident Care Director.
- Provide education and support to residents and their families on health-related topics.
Requirements:
- Graduate of an approved college or school of nursing.
- Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Minimum of one (1) year experience in a healthcare setting.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
