Position Overview:
The Licensed Practical Nurse is responsible for executing nursing tasks and providing patient care as directed by the healthcare facility's management team, including the Director of Nurses and/or Physician.

A Licensed Practical Nurse will engage in a range of responsibilities aimed at ensuring the comfort and care of patients in need of medical attention. The LPN is expected to apply specialized skills and sound judgment in delivering patient care, while also assessing and evaluating the effectiveness of care provided.

Key Responsibilities:
  • Deliver nursing care and procedures aligned with personalized treatment plans; administer prescribed medications; monitor and report on patient responses to treatments and any changes in health status; assist with personal needs, including daily living activities and hygiene, while providing health education to patients.
  • Accurately measure and document vital signs such as temperature, blood pressure, and pulse rates using standard medical instruments.
  • Under the supervision of a Registered Nurse, participate in the treatment of residents by administering medications, observing and reporting changes in their condition, and collecting specimens for laboratory analysis.
  • Prepare and apply basic dressings; assist with bathing and feeding residents; maintain cleanliness and organization of resident quarters by performing tasks such as making beds and changing linens.
  • Responsible for delivering professional nursing services through the nursing process of assessment, planning, implementation, and evaluation.
  • Monitor inventory levels to ensure adequate supplies are available to meet resident needs.
  • Communicate patient requirements to the medical team to facilitate coordinated care.
  • Engage in the resident admission screening process, which may involve travel as necessary.
  • Contribute to the development and enforcement of policies and procedures.
  • Participate in financial management, performance evaluations, maintain infection control standards, manage wounds, and engage in continuous quality improvement initiatives, as well as formal and informal education.
  • May provide oversight to subordinate professional, paraprofessional, and support staff in their resident care duties and assist in facility supervision.
Qualifications:
  • Must hold a current Licensed Practical Nurse license in New York State.
  • At least one year of recent nursing experience is required.
  • BLS certification is mandatory.
Additional Information:
Shift: Primarily evenings and nights available, including every other weekend and some holidays.
Hours: Full-Time
Contract Length: Up to 2 years.
