  • Discusses care plans with patients and their families, ensuring timely completion.
  • Monitors patient progress in accordance with care plans.
  • Adjusts care plans based on ongoing evaluations and patient conditions.
  • Ensures care meets established standards.
  • Facilitates effective discharge planning, addressing barriers and ensuring patient/family education.
  • Ensures patient safety in medication administration and procedures, preventing falls and infections.
  • Provides optimal pain management and addresses patient concerns regarding their condition and treatment.
  • Ensures patients understand their medications, including purpose, side effects, and administration instructions.
Documentation:
  • Maintains accurate, legible, and timely documentation.
  • Documentation reflects the nursing process and interventions undertaken.
  • Utilizes electronic systems effectively for optimal patient care.
Clinical Outcomes:
  • Communicates patient findings and progress with physicians and other healthcare team members.
  • Demonstrates competencies during the probationary period and through ongoing departmental evaluations.
  • Makes informed nursing decisions based on data interpretation and patient assessments.
  • Participates in departmental performance improvement initiatives.

Basic Qualifications:

Experience
  • Minimum of six (6) months of experience in an acute or sub-acute care setting within the last three (3) years.
Education
  • N/A
License, Certification, Registration
  • Registered Nurse License (California)
  • Basic Life Support Certification
Additional Requirements:
  • Basic arrhythmia knowledge required.
