Nurse Practitioner

Rochester Regional Health is seeking a skilled Registered Nurse to join our Home Care team. As a Community Health Nurse, you will provide nursing interventions, patient education, and care management to clients in their homes, promoting good health and one-on-one patient care.
About the Role:- Provide comprehensive assessments of clients' bio-psycho-social needs and develop individualized care plans.
- Perform skilled nursing care and prescribed treatments based on MD orders.
- Provide client and family education in accordance with client assessment and plan of care.
- Maintain effective communication with office and supervisory staff, as well as coordinate client services and referrals.
- Associate's Degree in Nursing required; Bachelor's Degree in Nursing preferred.
- 1 year of nursing experience preferred, with prior home health, clinical, and direct patient care experience.
- Ability to work independently and maintain effective communication.
