Nurse Practitioner

We are seeking a skilled and compassionate Nurse Practitioner to join our team at Rochester Regional Health. As a key member of our Home Care department, you will play a vital role in providing high-quality patient care and support to individuals in their homes.
Responsibilities- Conduct comprehensive assessments of patients' bio-psycho-social needs and develop individualized care plans.
- Provide skilled nursing care and prescribed treatments to patients in their homes, ensuring seamless continuity of care.
- Collaborate with the interdisciplinary health team to coordinate patient services and referrals, promoting efficient and timely care.
- Communicate effectively with patients, families, and healthcare professionals to ensure optimal patient outcomes.
- Document patient care and progress accurately and thoroughly, adhering to department and system protocols.
- Associate's Degree in Nursing required; Bachelor's Degree in Nursing preferred.
- 1 year of nursing experience preferred, with prior home health, clinical, and direct patient care experience a plus.
- Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
- A competitive salary range of $70,700.00 - $94,300.00.
