About the DepartmentThis department supports the collaboration efforts through patient and provider education, patient navigation, referring provider coordination, supporting consistent processes, and enabling strong and routine communication between teams.
Scope of Work- Coordinates advanced lung disease and transplantation of all recipients.
- Provides follow-up and care across the continuum from advanced lung disease to pre-transplant, during transplant and post transplant.
- Collaborates with members of the health care team to enhance patient care by developing, supporting and educating nursing staff.
- Makes clinical judgments based upon a synthesis of current research, concepts, principles, theories, and outcomes to solve complex problems.
- Facilitates advanced nursing care for patients before and after transplantation along the continuum of outpatient to inpatient as required.
- Coordinates evaluation process for pre-transplant patients and living donors (as applicable) to ensure effective use of hospital and/or clinical resources.
- Assesses post-transplant and post donation (as appropriate) organ function and general health status and collaboratively determines need for additional assessment or intervention.
- Participates in the development and implementation of protocol and standards of care for the transplant patient in collaboration with the multidisciplinary team.
- Required Bachelor's Degree Nursing
- Preferred Master's Degree Nursing
- 5 years of relevant experience nursing experience required
- Program development preferred
- MI Registered Nurse (RN) Required
- Basic Life Support (BLS) - AHA American Heart Association 90 Days required
- Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association Solid Organ Transplant - Adult 1 Year required
- Clinical Transplant Coordinator (CCTC) - ABTC American Board for Transplant Certification Solid Organ Transplant Upon Hire preferred
