We are seeking a skilled nursing professional to provide high-quality patient care in the comfort of their own homes. As a Home Health Care Provider with AdviaCare, you will be responsible for delivering individualized care to Medicare beneficiaries who require intermittent skilled nursing services following an in-patient stay.

Job Description

  • Provide skilled nursing services to patients in their homes, focusing on wound care, medication management, and disease education.
  • Evaluate patients' knowledge of home safety, medication management, and disease management, and determine their needs for community resources.
  • Collaborate with in-home therapists, physicians, other providers, caregivers, and family members to coordinate patient care.

As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families. We offer a competitive salary range of $80,000 - $110,000 per year, depending on experience, as well as excellent benefits, including medical insurance, 401(k) retirement plan, paid time off, and ongoing training and development opportunities.

Requirements
  • Michigan RN License
  • Michigan Driver's license and proof of insurance
  • CPR Certification
  • 1 year Home Health Experience Preferred; ER or Critical Care Experience a Plus
  • OASIS Experience Preferred
  • Critical Thinking and Task Skills Necessary
  • Computer Skills a Must
