Job Overview

We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our team in providing comprehensive home risk assessments to patients.

The Role:

You will conduct thorough adult/geriatric assessments, medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.

Responsibilities:

  • Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as necessary.
  • Work closely with Primary Care Physicians (PCPs) on patient education and provide follow-up support.
  • Provide services in various settings, including home visits, skilled facility visits, and telehealth visits.

Qualifications:

  • Master's degree required, OR commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
  • Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • Minimum 1 year experience as a Nurse Practitioner/Physician Assistant.
  • Strong computer skills and familiarity with EMR software.
  • Excellent verbal and written communication skills.
