Job Summary:
We are seeking a highly skilled Primary Medical Physician Assistant or Nurse Practitioner to join our team and contribute to the delivery of high-quality care to patients in short-term rehabilitation facilities, long-term care facilities, and sub-acute care settings.
Key Responsibilities:
- Conduct Comprehensive Patient Assessments: Perform thorough examinations, obtain medical histories, and assess the health status of patients.
- Diagnose and Manage Patient Conditions: Order, perform, and interpret diagnostic tests; prescribe medications and manage treatment plans effectively.
- Foster Collaborative Relationships: Consult with physicians on complex cases, coordinate specialty referrals, and provide guidance on preventive healthcare.
- Patient Education and Support: Develop personalized education plans, enhance patient understanding of treatment options, and promote healthy behaviors.
- Triage and Communication: Evaluate patient concerns, triage effectively, and ensure timely medical responses to optimize patient care.
- Documentation and Reporting: Maintain accurate medical records, develop status reports, and track patient progress towards goals.
- Team Collaboration: Participate in interdisciplinary meetings to discuss patient care, treatment modifications, and progress toward shared goals.
Requirements:
- Licensure and Certification: Active Physician Assistant or Nurse Practitioner License required.
- Experience: Minimum two years of experience as a PA/NP in an acute, subacute, or rehabilitation setting preferred.
Benefits and Compensation:
We offer a competitive salary range of $135,000 - $170,000 per year, along with a comprehensive benefits package including:
