Responsibilities- Develop and implement comprehensive care plans tailored to each patient's unique needs
- Provide hands-on care, management, and evaluation of the care plan, ensuring seamless coordination with other healthcare professionals
- Administer ongoing care and case management, providing necessary follow-up as directed by the Clinical Manager
- Collaborate with patients, families, and healthcare teams to achieve optimal resolution of needs and problems
- Graduate of an accredited nursing program
- Current licensed Registered Nurse (RN) in the state of practice
- Experience as an RN with one year of experience in an acute-care setting within the last 24 months or six months of Home Health experience within the last 12 months preferred
- $65,000 - $85,000 per year, depending on experience
