About the Job

At All Medical Personnel, we are seeking a highly skilled Physical Therapy Assistant Specialist to join our team. In this role, you will have the opportunity to work with patients in their homes, providing them with the care and support they need to recover from illnesses or injuries.

Job Responsibilities

  • Collaborate with the interdisciplinary team to develop holistic plans of care that focus on achieving patient physical therapy goals.
  • Instruct, coach, guide, and intervene to support patients and families in achieving the best possible outcomes while remaining in their communities.
  • Promote optimal health, well-being, and quality of life for home health patients and their families by providing skilled, intermittent physical therapy in the patient's home.
  • Practice independently in the patient's home using excellent evidence-based practice for assessment, clinical decision making, clinical practice, and coordination of care across the continuum.
  • Act as case manager and team lead for patients requiring home health therapy services, when nursing is either not involved in the case or physical therapy is the primary patient need.
  • Assess the patient and collaborate with the interdisciplinary team and primary care provider to develop the patient-centered plan of care.
  • Provide physical therapy treatment to the patient/family/caregiver as ordered.
  • Complete accurate clinical documentation to effectively support service provided within specified timeframes.
  • Provide physical therapy to pediatric, adolescent, adult, and geriatric patients based on principles of growth and development and life stages.
  • Maintain communication regarding patients with the case manager and other disciplines via care conferences or other appropriate methods.
  • Reports acute medical problems and condition changes to the primary care provider.
  • Provides supervision to Physical Therapy Assistants, Home Health Aides, and students in the clinical setting.
  • Completes regulatory required assessments within specified timeframes.
  • Completes medication reconciliation at specified timeframes.
