Midtown Community Health seeks a detail-oriented and empathetic Patient Care Coordinator to join our team. As a key member of our primary care staff, you will coordinate patient care activities, ensure seamless transitions, and promote excellent patient outcomes.

Job Description:

The Patient Care Coordinator will work closely with medical providers, nurses, and other healthcare professionals to ensure effective patient care planning, implementation, and follow-up. You will be responsible for coordinating patient transfers, communicating with family members, and providing emotional support to patients and their loved ones.

Responsibilities:

  • Coordinate patient transfers between departments and facilities.
  • Communicate with family members and caregivers regarding patient status and care plans.
  • Provide emotional support to patients and their loved ones during challenging times.
  • Assist with discharge planning and post-acute care arrangements.

Requirements:

  • Diploma or certificate in healthcare administration or a related field.
  • Previous experience in healthcare or social work preferred.
  • Ability to effectively communicate verbally and in writing in English and Spanish.
  • Basic Life Support (BLS) certification preferred.
