Job Summary

We are seeking a skilled and compassionate Registered Nurse (RN) to join our team of home health professionals. As a Weekend Registered Nurse, you will provide high-quality patient care and support to individuals in the comfort of their own homes.

Responsibilities
  • Perform comprehensive patient assessments and develop individualized plans of care in collaboration with the multidisciplinary care team.
  • Maintain continuity of patient care by communicating effectively with all team members providing care to the patient.
  • Provide patient episode management per protocol and complete documentation timely and accurately according to industry standards.
  • Educate patients, caregivers, and family members on aftercare or ongoing care of the patient.
Requirements
  • Current and unencumbered RN license.
  • 1+ year(s) of Certified Home Health experience as an RN.
  • OASIS experience.
  • Strong assessment, critical thinking, and communication skills.
  • Current CPR certification.
  • Comfortability using technical tools and software/EMR for visit documentation.
  • Reliable transportation, current driver's license, and car insurance coverage.
  • Proof of Immunization/Exemption for Flu and COVID-19.
  • TB Test or Chest X-Ray.
