Key Responsibilities:
• Conduct thorough physical and psychosocial assessments of patients to identify their needs and develop personalized care plans
• Establish and implement care plans that address the patient's medical, emotional, and social needs
• Collaborate with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes
• Educate patients and families on procedures, wellness practices, and self-care techniques to promote independence and empowerment
• Monitor patient progress, adjust care plans as needed, and ensure timely interventions to prevent complications
• Maintain accurate and detailed records of patient interactions, care plans, and outcomes
• Stay up-to-date with the latest clinical guidelines, research, and best practices in home health care
• Participate in ongoing education and training to enhance professional skills and knowledge

Requirements:
• Current licensure as a Registered Nurse (RN) in the state of practice
