Job Summary:
As a Weekend Home Health Nurse with ECHN, you will provide high-quality, patient-centered care to individuals in their homes, ensuring their comfort, safety, and well-being. This role requires strong assessment and planning skills, as well as the ability to develop and implement effective care plans that meet the unique needs of each patient.

Key Responsibilities:
• Conduct thorough physical and psychosocial assessments of patients to identify their needs and develop personalized care plans
• Establish and implement care plans that address the patient's medical, emotional, and social needs
• Collaborate with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes
• Educate patients and families on procedures, wellness practices, and self-care techniques to promote independence and empowerment
• Monitor patient progress, adjust care plans as needed, and ensure timely interventions to prevent complications
• Maintain accurate and detailed records of patient interactions, care plans, and outcomes
• Stay up-to-date with the latest clinical guidelines, research, and best practices in home health care
• Participate in ongoing education and training to enhance professional skills and knowledge
• Contribute to a positive, supportive team environment that fosters open communication, collaboration, and mutual respect

Requirements:
• Current licensure as a Registered Nurse (RN) in the state of practice
• Valid driver's license and reliable transportation
• Strong assessment, planning, and implementation skills
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong problem-solving and critical thinking skills
• Proficiency in electronic medical records and other relevant software systems
• Ability to maintain confidentiality and handle sensitive patient information with discretion
