Job Description

The Medical Care Coordinator is responsible for ensuring that patients receive high-quality, comprehensive care by facilitating effective communication between healthcare providers, patients, and families.

Duties and Responsibilities
  • Manage patient care plans, coordinate appointments, and schedule medical tests as needed.
  • Communicate with patients, families, and healthcare providers to gather and share relevant information.
  • Provide education and support to patients and families on managing chronic conditions and adhering to treatment plans.
  • Collaborate with interdisciplinary teams to identify and address patient needs.
Qualifications
  • Associate's degree in healthcare-related field or equivalent experience.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Proficiency in Microsoft Office software.
Compensation and Benefits
  • Salary: $50,000 - $60,000 per year.
