Job Overview

We are seeking an experienced Clinical Care Coordinator to join our team. The successful candidate will be responsible for providing comprehensive care to patients and their families, collaborating with healthcare professionals, and ensuring seamless care coordination.

Key Responsibilities
  • Patient Assessments: Perform thorough assessments to identify patient needs and develop personalized care plans.
  • Care Coordination: Collaborate with healthcare teams to ensure timely and effective care delivery.
  • Caregiver Education: Educate patients and caregivers on disease management, symptom control, and treatment options.
  • Documentation: Maintain accurate and detailed records of patient interactions and care activities.
  • Communication: Effectively communicate patient needs and care plans to healthcare teams and family members.
Requirements
  • Experience: Minimum 2 years of experience in clinical care or a related field.
  • Certifications/Licenses: Current nursing licensure and certification in palliative care.
  • Skills: Excellent communication, empathy, and problem-solving skills; ability to work independently and as part of a team.
