Physician Assistant/Nurse Practitioner Opportunity

The Opportunity
Our Extended Care team provides episodic and chronic care to patients in skilled nursing or long-term care facilities across the North Shore. We closely monitor patient progress, coordinate with specialists, and keep primary care physicians and families informed. When patients are ready to return home, we coordinate a plan of care with their primary care physician for a smooth transition.
Responsibilities
As a member of our Extended Care team, you'll be responsible for:
- Assessing patient health needs, performing physical examinations, and managing therapeutic regimens
- Caring for patients with acute and chronic life-limiting illnesses
- Managing and evaluating new and follow-up patients with physician input
- Assuming all SNF/LTC care management responsibilities
- Facilitating communication with patients, families, and referring physicians
The Schedule
This is a full-time, 40-hour position. You'll see 10-11 patients per day and be available to respond to pagers within 15 minutes between 8:00am and 5:00pm, Monday-Friday. You'll also be on-call one night per week and two weekend days per month, compensated accordingly.
Qualifications
We're looking for a certified Massachusetts Nurse Practitioner or Physician Assistant with a Master's degree in nursing or physician assistant studies. Prior SNF/home care experience is preferred.
