Post Acute Care Services Manager

Ascension Saint Agnes Hospital is seeking a skilled RN Manager to oversee the integration and management of Post Acute Care, Palliative Care, and Transitional Care Management services. The ideal candidate will have substantial experience in a healthcare setting, specifically with exposure to post-acute care, palliative care, or transitional care. The successful RN Manager will leverage their clinical expertise to conduct chart reviews, monitor readmissions, and ensure high-risk patient groups receive appropriate care across settings.
Responsibilities
- Manage assigned Registered Nurse (RN) case managers and related activities to ensure effective patient care.
- Communicate with administration and medical staff on hospital admissions, discharge planning, and length of stay.
- Develop and implement staff schedules to ensure optimal unit coverage and workload distribution.
- Cover units as needed and serve as a resource for complex case management issues.
Requirements
- Registered Nurse licensure from the Maryland Board of Nursing required.
- Case Manager certification from the Commission for Case Manager Certification preferred.
- 3 years of experience in a healthcare setting with leadership experience required.
