RN Patient Care Manager

We are seeking a dedicated and compassionate Patient Care Manager to join our team at Ochsner Home Health of Raceland. As a Patient Care Manager, you will play a vital role in ensuring the delivery of high-quality patient care in the comfort of their own homes.
Key Responsibilities:- Supervise and coordinate clinical services to ensure seamless patient care
- Provide clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care and in coordination with other healthcare team members
- Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start of care visits
- Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals
- Ensure patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders
- Current RN licensure in the state of LA
- Current CPR certification required
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
